• name,
• surname,
• personal identification number,
• phone number,
• the year for which the certificate is required. 

If you do not have the opportunity to submit signed application by e-mail, please call and agree on the time and place to submit the application to the authorized person. Along with a free-form application that must include your "handwritten signature", provide a photo of your ID with your own photo. It is best that both your face and your passport or ID card appear in one photo.
